Email Consent Form

Email Communication Consent Form

At Truwell Medical Services, we are committed to facilitating secure, efficient, and convenient communication between our clinical team and our patients. As healthcare technology evolves, email serves as a valuable tool for sending administrative updates, appointment confirmations, and routine health notices. However, because standard email is not an entirely secure medium for transmitting sensitive clinical details, we require your explicit consent before communicating electronically.

Please review the terms, risks, and guidelines below before completing our electronic communication consent form.

Benefits & Inherent Risks of Email Communication

  • Enhanced Convenience: Allows for rapid transmission of appointment reminders, scheduling updates, and administrative paperwork.

  • Potential Security Risks: Standard, unencrypted email messages can potentially be intercepted, viewed, stored, or altered by unauthorized third parties during transmission.

  • Not for Urgent Care: Email correspondence is never monitored 24/7 and must never be used for medical emergencies or urgent health symptoms. For emergencies, dial 911 or visit your nearest hospital emergency department.

  • Medical Record Integration: Important communications or notices sent via email may be documented and saved within your permanent electronic medical record (EMR).

Patient Acknowledgement & Guidelines

  • Voluntary Consent: Giving consent to receive email communications is completely voluntary and is not a prerequisite for receiving medical care or physician consultations at our clinic.

  • Right to Revoke: You maintain the absolute right to withdraw or revoke your email consent at any time by notifying our administrative office in writing.

  • Patient Responsibility: You are responsible for ensuring that your provided email address is accurate, secure, and accessible only by you, and you must notify the clinic immediately if your contact details change.
